It keeps taking me to MAK's Kodak gallery!!  Shocked I even rebooted Firefox and still Carrie's link is MAK's album.

I tried without success to upload the photos and captions from iPhoto to an album in my premium Kodak Gallery http://classic.kodakgallery.com/shaky

However, Carrie was able to upload the photos to a regular Kodak Gallery album and then type in the captions again. Here is the link. No login required.
